Providing Assistance and Support to Seniors in Transition

Through the PASST Program, qualified seniors will receive: 

  •  Vital Nutrition 
    • Two meals a day for a minimum of two weeks to seniors following hospital and other inpatient programs who are discharged for a minimum of two weeks. Each of our meals is freshly prepared daily, is low in sodium, and provides a third of the daily required dietary allowance. A healthy diet helps strengthen the immune system, shorten hospital stays, and prevent or delay chronic diseases. *Elderly caregivers may also be eligible to receive meals for up to two weeks.
  •  Personalized Case Assistance
    • SSA professional staff, including LMSW staff members, will meet with patients and caregivers to assess needs, establish goals, and identify available resources to help patients achieve those goals.
